We have an opening for a School Based Community Health Worker. The CHW for School Based Health is the main administrative support employee for a SBHC and works in conjunction with the SBHC staff to establish relationships with student population, school staff, parents, and the community in order to increase awareness of School Based Health in the Peabody and Salem Communities. The CHW for School Based Health is an essential member of the SBHC team, often the first person with whom a potential student or parent will interact and is responsible for coordinating daily activities and functions of the SBHC. The SBHC CHW is responsible for the day to day flow of patient enrollment, registration and processing during scheduled visits and walk in requests for appointments. In addition to provided needed administrative support to the School Based Health Center, the CHW will establish and maintain Youth Advisory Councils in order to engage youth in the school based health model and will empower students to advocate for health and community issues important to them  

As a team member at our health center, you will work with staff who are racially, ethnically and culturally diverse, so being culturally competent is required.  A sense of humor is also very important!  


  • Enroll, register and process patients using the electronic medical record. This may include additional registration forms, encounter forms as required by NSCHI or other funders and partners
  • Monitor and facilitate patient flow, to assure efficiency for patients and providers.
  • Assist SBHC staff with promoting health center services to greater school and community
  • Create and facilitate SBHC Youth Advisory Councils (YAC)
  • Collaborate with school and community groups on projects related to school health and wellness.
  • Assist Director of School Based Health in finding funding sources/ grant opportunities for the program.
  • Connect with community leaders and professionals to conduct enrichment workshops/trainings for students.
  • Plan field trips and educational experiences for students.
  • Order and restock health center administrative supplies
  • Maintain confidentiality in accordance with NSCHI Confidentiality Policy.
  • Contribute to program planning, development and evaluation.
  • Attend relevant meetings and trainings.
  • Perform word processing and data entry duties as needed. Prepare reports as requested by other SBHC or NSCHI staff.
  • Communicate with students and parents as requested by providers to reschedule appointments, reschedule lab tests, arrange referrals, and request insurance information.
  • Transport lab samples to PFHC or SFHC as requested.
  • Assist students with no insurance to appropriate services in SBHC, NSCHI or school.
  • Demonstrates excellent customer service skills, communication skills and be able to maintain confidentiality.
  • Demonstrates attention to detail, flexibility and ability to multi task in a fast paced environment.
  • Assists uninsured students in determining eligibility and enrollment into a state health insurance plan


  • SBHC experience preferred
  • Community health center experience preferred
  • Excellent customer service, communication and organizational skills
  • Experience in community outreach and program support
  • Proficient in Microsoft Office
  • Experience with Virtual Gateway, health insurance and enrollment, medical referrals
  • Preference will be given to bilingual/bicultural individuals who can speak Spanish and/or Portuguese
